Non-Medical Support for Patients at Home

Dignity Extensivists of Central Texas recognizes that medical care is only one part of a successful recovery at home.

Beyond medical care

Support for Everyday Life at Home

Many patients and families also need help with everyday activities, personal care, mobility, companionship, meal preparation, and other non-medical needs.

To help address these needs, Dignity Extensivists partners with Dignity Transition Care, a nurse-led provider of non-medical home-care and Personal Assistance Services (PAS).

Medical care + non-medical support

How Dignity Extensivists and Dignity Transition Care Work Together

A patient's recovery often involves needs that go beyond medical treatment. For example, a patient returning home after hospitalization may need both physician-led medical care and non-medical home support.

Together, these services can help address both the patient's medical needs and the practical challenges of recovering at home.

Our goal

To help patients and families navigate the transition from hospital to home with greater support and continuity.

Live alone or need additional companionship
Have family caregivers who need respite
Need help with meals or light household tasks

Each patient's needs are different

Dignity Transition Care works with families to develop an appropriate non-medical care plan based on the individual's needs and preferences.

Important distinction

Non-Medical Care Is Different From Medical Care

Dignity Transition Care provides non-medical home-care services. These services are different from the physician-led medical services provided by Dignity Extensivists.

For medical concerns, patients should continue working with their physician and healthcare team.

Private caregivers do not replace:

A primary care physician
A specialist
A physician providing transitional medical care
Skilled medical nursing services
Emergency medical services

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
