Supporting You After You Leave the Hospital

Dignity Extensivists is a physician-led post-acute and transitional medical care organization extending care from the hospital to the home and beyond.

Recovering from a hospital stay doesn’t end at discharge. Our team follows you home, catching small problems before they become emergencies and keeping your primary care provider in the loop the whole way.

FAQ

Answering Some of Your Questions

What happens during a transitional care visit?+

Your visit may include a review of your hospital stay and discharge instructions, a medication review to prevent errors or side effects, monitoring of your symptoms and recovery progress, labs or imaging if needed, coordination of specialty referrals, help arranging primary care follow-up, and education on what to watch for and when to call.

Do I have to come into an office?+

No. Our clinicians come to you, most visits happen in your home, and some follow-ups can be done by secure video when appropriate.

What is Remote Patient Monitoring, and do I need it?+

Remote Patient Monitoring uses connected devices to track readings such as blood pressure, blood sugar, oxygen level and weight from home, so our team can spot changes early. Whether it is right for you depends on your condition and recovery, we will talk it through with you.

What areas do you serve?+

We serve Austin, Leander, Cedar Park, Bee Cave, Dripping Springs, San Marcos, Buda, Manor, Pflugerville, Round Rock and Georgetown.

Will you communicate with my primary care doctor?+

Yes. Our care is designed to complement - not replace - your established primary care physician and specialists. We keep them informed throughout your recovery.

What if I don’t have a primary care provider?+

We can still care for you after discharge, and we can help you arrange primary care follow-up so you have a doctor for the long term.

How do I make a referral as a hospital or facility?+

Call (512) 503-1169 or email [email protected] with the patient’s information, and our team takes it from there.
