Personalized Medical Care After You Leave the Hospital

Leaving the hospital is an important transition in your healthcare journey. Patients and families may suddenly have new medications, follow-up appointments, new diagnoses, changes in mobility, and questions about what comes next.

Dignity Extensivists of Central Texas provides physician-led transitional medical visits designed to help patients navigate this period with greater coordination, convenience, and confidence.

Our concierge transitional care visits are focused specifically on the period following hospitalization and are designed to complement, not replace, your established primary care physician and specialists.

The service

What Is a Concierge Transitional Care Visit?

A Concierge Transitional Care Visit is a physician-led medical visit designed for patients who have recently been discharged from a hospital or other acute-care setting and need additional medical attention during the transition to home or another care setting.

Depending on the patient's individual needs, the visit may include review of the recent hospitalization, medication reconciliation, assessment of current symptoms and medical concerns, review of discharge instructions, coordination with existing healthcare providers, and development of an appropriate follow-up plan.

Where Does the Visit Take Place?

Our Concierge Transitional Care Visits are designed to provide convenient medical care during the transition after hospitalization.

Depending on the patient's circumstances and eligibility, visits may be provided in the patient's home.

Our goal is to make the transition as convenient as possible while maintaining appropriate medical coordination with the patient's existing healthcare providers.

Working together

Your Existing Primary Care Physician Remains Part of Your Care Team

Dignity Extensivists is not intended to replace your established primary care physician or specialists.

Instead, our transitional care model is designed to work alongside your existing healthcare team during the period immediately following hospitalization.

With appropriate patient authorization, we can help communicate relevant information and recommendations with your primary care physician and specialists.

Our goal is simple

We help connect the pieces of your healthcare journey.

Laboratory services

Convenient Options for Required Laboratory Testing

When laboratory testing is clinically appropriate, patients have options.

Option 1

Visit a Laboratory

Patients may choose to visit a laboratory location themselves for ordered testing.

Our current laboratory partner is Quest Diagnostics.

Option 2

Have Blood Drawn at Home

For patients who prefer the convenience of having laboratory specimens collected at home, a separate private-duty nurse visit may be arranged for blood collection.

The nurse can collect the appropriate specimen at the patient's home and deliver the sample to the laboratory for processing.

Please note: The private-duty nursing blood-draw visit is a separate service and may involve a separate fee.
